To access this tab: Click the Incidents icon
on the toolbar, then click Add or Change. Select the Person tab.
Use this tab to record the details of the person who was hurt, could have been hurt or was involved in the incident or accident.
Note: Some fields are required for OSH Serious Harm Accident Notification and Non-Serious Accident records if the incident involved harm. If the incident involves only no-harm loss or damage, any requirement is up to your company's accident reporting procedures.
Staff ID Code (employees only)
If the person involved or injured is an employee, their ID code is entered here using the lookup button. Selecting an employee fills in most other fields on this tab from the employee record.
If the person is not an employee leave this field blank.
Start date (employees only)
This is the date the person started work at the organisation.
Job Title (employees only)
This is the person's job title.
Contractor (contractor only)
This is the name of the supplier for whom the person works.
Contractor person (contractor only)
This is the name of the person involved in the incident.
This is the person's first name.
This is the person's last name.
This is the person's residential address.
This is the person's residential suburb.
This is the person's city of residence.
This is the person's date of birth. This is required for OSH reporting.
This is the person's gender, either Female or Male.
This is the length of time the person has been employed by your company. Select the period using the option buttons provided. If the person is not an employee, select the Non-Employee option.
This information is required for Accident reports. Select one of the options provided.
